Define severe diarrhea, the pill becomes ineffective when you have 6+ trips to the bathroom of completely liquid stool in a certain amount of time. Consult a pharmacist for advice on anti-diarrhea meds like immodium or peptobismol

If you feel like you’ve had 6+ trips of complete liquid stool then you should follow missed pill instructions.

5/6+ episodes of completely liquid stool within 24 hours of taking a pill will count as a missed pill. If it’s not that severe then you should be okay. Multiple episodes of completely liquid stool every day for several days in a row will also mean your protection has been affected, treat it as if you’ve missed multiple pills. If you’re unsure you’re best to assume your protection has been affected and use extra protection until you’re sure your pill is fully effective again. If you take an extra pill to make up for the missed pill but continue to have diarrhoea then assume taking the extra pill didn’t help. If you take an extra pill outside of your pill’s allowed time frame to make up for the missed pill then you still need to treat it as a missed pill.

Theoretically no, if you're having ongoing severe diarrhea then your body is not properly absorbing anything so an extra pill will also not be absorbed. Act as if the pill is missed.
