What are the chances of the pill failing even if you take it everyday? I take the combined pill and have never missed one and take it within the same hour everyday and still use a condom, however the condom has broken on some occasions and i cant help but be somewhat concerned.

0.3% in other words one third of one percent. The pill is 99.7% effective when used perfectly.

^ I want to add that if you have the condom break on some occasions it’s very likely they might not be the correct size or aren’t being used correctly.
